The final coroner's report on Cory Monteith's death was released yesterday (3rd October). The 31-year-old Glee star was found dead in his Vancouver hotel room on 13th July where he had overdosed on a mix of heroin and alcohol. The report provides further details into the cause and the scene of his death.

It seems Monteith had been shooting heroin and drinking champagne in the hours before his death. His body was found in his room at the Fairmont Pacific Rim Hotel in Vancouver, shortly before noon after the actor failed to check out. According to the report (obtained by TMZ) two empty champagne bottles were found in the room in addition to drug paraphernalia. The latter included a "spoon with drug residue and a used hypodermic needle."

Monteith's cause of death was listed as "mixed drug toxicity, involving intravenous heroin use combined with the ingestion of alcohol." Although the coroner noted Monteith "had a history of illicit drug use with intermittent periods of rehabilitation and of abstinence from drugs," it may be his tolerance for heroin had lowered considerably since his stay in rehab in April 2013.

The report comes in the wake of the tribute paid to Monteith as the Emmy Awards ceremony on 22nd September. His Glee co-star Jane Lynch presented the tribute in which she said Monteith possessed "warmth" and "charm". She continued by saying that, whilst playing quarter-back Finn Hudson, Monteith "had a star quality and a genuine sweetness that made it impossible not to fall in love with him." Lynch further stated "His death is a tragic reminder of the rapacious, senseless destruction that's brought on by addiction."
