In years past Dr Heiter had been one of the best surgeons of his kind, separating siamese twins was his speciality. Since his retirement, Dr Heiter spent most of his time at home. When two American friends who're on a roadtrip through Europe find themselves in a broken-down car, the only choice is to get out and walk.

Looking for help, the closest house they fall upon is the one that belongs to the doctor. Asking him to call breakdown assistance, he informs the the girls they wont be able to come out until the morning, they take up the doctors offer for them to stay the night.

When they wake up, they find themselves under the house in a makeshift clinical ward/operation theatre. As it becomes clearer just how twisted Dr Heiter's mind is, they know the girls may never leave the property without the doctor taking his knife to make a few alterations of his own to their bodies.
